The diagnosis of lumbar back pain is difficult and uncertain due to various conditions that can occur with this problem. Effective management of back pain depends on identifying what type of problem of back pain is present, and many people have suggested that there are many subtypes of pain backs that must be identified before treatment can be targeted. Changes in diagnoses for back pain and associated symptoms include pain postural; trigger point pain, compression of the nerve root, neuropathic pain, the facet joint pain, pain related to disc and stenosis lumbar.
The spinal facet joints, intervertebral discs, muscles and ligaments are all potential sources of mechanical back pain, of pain from injured or damaged tissues, and transmitted by the nervous system. When the nervous system is damaged or hurt it can begin to produce pain itself, leading to the production of so-called neuropathic pain. Diagnostics of the conventional type of pain is post-shingles pain, phantom pain, nerve root damage pain and diabetic neuropathy pain. Patients suffering badly with this type of pain and unpleasant it is difficult to treat.
A recent study by researchers at Massachusetts General Hospital in Boston and Addenbrooke's Hospital, Cambridge, United Kingdom, investigated this problem. They recognized that the assessment by taking a score of pain intensity does not reflect not the reality of the complex nature of the grief process by which pain is generated. They aim to design an assessment that takes account of these complexities, allowing clear identification of the diagnosis and treatment can potentially more accurate. They have developed a standardized tool to use in the assessment of chronic pain in order to define different subtypes of pain.
130 people suffering from peripheral neuropathic pain and 57 persons suffering from mechanical back pain were interviewed and given a standardized assessment. An interview with 16 questions were then applied followed by a specific set of twenty-three physical tests. A list of words applicable to descriptions of pain were provided and patients were asked to indicate which ones most accurately described their pain. In patients with chronic pain often have impaired the ability to sense touch, vibration and pin prick stimuli so that the ability to discriminate these sensitivities is tested.
In patients neuropathic pain, it was possible to identify six sub-groups and among patients without neuropathic two additional subgroups were noted. Researchers were also able to distinguish between questions 6 and 10 physical tests that were best suited to make the most accurate discrimination between subtypes of pain. Testing tool hundred thirty seven patients also allowed researchers to see that it worked effectively and that the acceptability of patients was good. A particular group of sub-types of neuropathic pain can be elucidated by a relatively small number of signs and symptoms that were not related to the surrender causal conditions.
Recording of symptoms was less sensible to distinguish the nature of neuropathic pain that physical examination. The quality of pain was less often noted, and the puncture tests more useful. Researchers have tried to link the subtypes of pain with the biological mechanisms underlying specific spontaneous pain with a burning nature related to spontaneous discharges in the heat sensitive nerve pain and pain associated with the increased sensitivity of cells to brushing in the dorsal horn of the spinal cord.
Physical examination was more sensitive in the delineation of diagnoses of neuropathic pain of the registration types and nature of symptoms. The qualities of pain were less useful and prick tests more useful. Researchers have attempted to link the neural mechanisms underlying the subtypes pain. Nerve pain, heat sensitivity were related to the burning pain of a spontaneous type and increased sensitivity of spinal cord cells in the dorsal horn was associated with increased pain on brushing the skin.
